Diabetes Prevention Program

Information for GPs: Purpose of study
More than one in five (20.1%) of Victorians have IFG or IGT and many of these will progress to overt diabetes. The Diabetes Prevention – go for you life program is funded by the Victorian Department of Human Services. The aim of the Program is to identify people with “pre-diabetes” (impaired glucose tolerance (IGT) and impaired fasting glucose (IFG)) and offer the Healthy Lifestye Course to support lifestyle changes to reduce the risk of progression to diabetes.

The Healthy Living Course is run at Whitehorse Community Health Service, 43 Carrington Road, Box Hill 3128. Tel. 9890 2220

Course criteria:

FBG—5.5-6.9 mmol/L or OGTT indicates IGT or IFG
Cost to patient for 6 session Healthy living course—$30 (fees can be waived in situations of financial difficulty)
Patient follow-up conducted by Whitehorse Community Health Service, with feedback provided to the referring GP
Electronic or paper-based referral forms—continue to use these
Courses in English & Chinese languages
